Neighbourhood health

We provide neighbourhood health information to help people working in health and social care across York, to make better, more joined-up decisions about the needs of local communities.

Neighbourhood-level data

To help Integrated Neighbourhood Teams (INTs) to understand what’s happening in their area, we collect and share data at a neighbourhood-level, covering:

  • population health
  • service use
  • social factors

By bringing together information from the NHS, local councils, and other partners, we can identify where people might need more support, spot gaps in services, and plan care more proactively. Essentially, it’s about using data to guide smarter, more coordinated decisions that improve people’s health and wellbeing locally.
